Connect2Care Prevent: A Statewide Diabetes Screening and Intervention Initiative

Connect2Care Prevent is a digital outreach initiative designed to engage Oklahoma Blood Institute donors who may be at elevated risk for type 2 diabetes based on blood sample testing. Building on the successful Connect2Care model for prostate cancer early detection, the program provides scalable digital education, risk stratification, navigation, and follow-up support to promote diabetes prevention, early care, and long-term self-management while supporting the Harold Hamm Diabetes Center's research infrastructure.
